NCCMI Required Acceptance Form and Covid Waiver Form

Download the forms by clicking HERE for the Acceptance Form and HERE for the Covid Waiver Form. Have both student and parent/guardian carefully review these forms. In order to participate in the NCCMI program, both student and parent/guardian must sign and return both the forms. This signed forms and a check for the balance of tuition due should then be mailed to: NC Chamber Music Institute, 3131 Ashel Street, Raleigh, NC 27612

These must be signed by every student and at least one parent/guardian, and submitted to NCCMI with full program tuition payment before acceptance in the NCCMI program is considered complete.
